Please note: For long-term stays of more than 90 days for medical treatment in Germany, please contact us.

Required documents

Please bring the following documents to your appointment at the VFS Global Visa Application Centre. If not mentioned otherwise, copies of the original documents are sufficient. Please note that the visa section does not return original documents, if you do not also provide a copy.

Documents in languages other than German, English or French must be translated.

Further documents may be requested in individual cases:

One application form (Schengen VIDEX) duly completed and signed.


1 recent biometric passport photo, 35 x 45 mm, no older than three months. Digitally altered passport photos cannot be accepted. Please ensure that your photo adheres to German biometric requirements and is taken in front of a white/bright background.

Do not stick or staple the photo to the application form.

Original passport or travel document valid for at least 3 months after the intended date of departure from the territory of the Schengen area. The passport/travel document must contain at least two blank pages and must have been issued within the last 10 years. Please attach 1 photocopy of the passport data page to your application.

Proof of legal residence in Qatar, valid for at least 3 months beyond the intended date of departure from the Schengen territory.

Applicants must prove that they are in possession of adequate and valid travel medical insurance, covering emergency medical, hospitalisation and repatriation (including in case of death). The minimum cover should be of 30,000 EUR. This insurance must be valid for the entire Schengen area and throughout the duration of the stay.

Applicants for a uniform visa for more than two entries (multiple entries) shall be in possession of travel medical insurance covering their stays; at the time of application, they need to prove that they have a travel medical insurance covering the period of their first intended visit, and sign the related statement in the application form.

Appointment letter from the hospital or doctor in Germany stating hospital admission date and approx. duration of the treatment

For QAT citizens:

Letter from Ministry of Public Health stating coverage of medical treatment and related expenses for patient and escorts

For non-QAT residents and/ or self-paying applicants:

Cost estimate and proof of payment issued by the hospital/ doctor for the intended treatment

Only applicable for non-Qatari residents

The employer/ sponsor letter must be submitted in original and must:

  • be addressed to the German Embassy and signed by a legal representative of the company as per company registration
  • mention name, postal address, telephone number, position of employee
  • contain applicant's salary
  • contain a copy of company registration/computer card showing legal representative(s)

Only applicable for non-Qatari residents

  • Original bank statements of your personal current account ( last 3 months and not older than 2 weeks prior to the day of submission)

NOTE: In case bank statements are provided by a direct relative (parent/child) or spouse, a birth certificate or marriage certificate (translated into English) must be submitted for proof of relationship

  • Stamped original and copy of employment contract registered with the competent Qatari labor authorities
  • Duly filled out letter of guarantee signed by sponsor
  • Passport copy of sponsor and copy of his/her Schengen visa if applicable
  • Letter of MOI (immigration) stating date of entry into Qatar (proof of minimum residence of 1 year)

Important Notice - Sponsor must be present at VFS to sign the guarantee form in front of VFS staff and present original QID and passport!
